The Access Coordinator II (AC II), with direction and supervision from the Dennis Developmental Center (DDC) leadership and in collaboration with other members of the DDC Care Coordination team performs a broad variety of administrative and clerical support functions related to the referral, intake, and triage functions of the Care Coordination Office (CCO) to ensure timely and appropriate Patient and Family Centered Care (PFCC).

The AC II is expected to be knowledgeable of clinic-specific processes and display an understanding of how the CCO role fits into the mission of the clinic and the workflow in support of the mission. The incumbent will be the primary point of contact for access to DDC services to both patient families and referring clinics.  The AC II interprets agency/institution administrative directives, policies and procedures to ensure consistent application. Provides a high level of customer service to external and internal constituents. Duties and responsibilities include, but are not limited to:  

  • Answering and processing all incoming phone calls to the Care Coordination Office.
  • Retrieving and processing incoming faxes and emails to the Care Coordination Office.
  • Obtaining and logging Medicaid referrals from PCP offices.
  • Mailing out Welcome and Appointment Packets and processing completed packets that have been returned.
  • Monitoring and follow-up for internal tracking of Appointment packets and scheduling process.
  • Patient registration into an electronic health record (EHR).
  • Scheduling patients in the practice management software application.
  • Mailing out Appointment letters and parent forms.
  • Collaborate and coordinate administrative and clerical duties with the CCO program coordinator to ensure timely and appropriate workflow in the CCO office. 


High school diploma/GED; plus 2 years customer service or healthcare experience. Computer/basic keyboard skills, telephone etiquette skills, and general knowledge of office machines including printers and scanners required. Knowledge in basic medical terminology preferred, medical terminology competent within 3 months of hire.



Preferred experience:

Registration experience is preferred.
