  • Overview

    The Licensed Certified Social Worker (LCSW) - Behavioral Health Consultant (BHC) serves as behavioral health provider within the Patient Centered Medical Homes (PCMH) in the UAMS primary care clinics. This position is responsible for providing direct patient/family care, as well as engaging in program development and interdisciplinary team development and additional departmental duties.  The BHCs function as an integrated member of the PCMH and assist in managing the overall health of the enrolled population through improved recognition, treatment, and management of psychosocial/behavioral problems and psychosocial aspects of medical conditions.

    Responsibilities

    Consultation

    Completes brief consultations with individuals, couples and/or family consultation with patients, including: functional and strength-based assessment and diagnosis;

    Administers and interprets brief objective symptom measures;

    Provides psychoeducation for patients and their support systems;

    Provides medication adherence counseling and disease self-management counseling;

    Interventions

    Demonstrates competency in providing cognitive-behavioral and other evidence-based/evidence-informed brief interventions to individuals, families, couples, and groups with behavioral health, substance abuse, and chronic/acute physical health problems in a primary care setting.  

    Consults with healthcare team members to enhance understanding of the patient, provides decision support for treatment planning, and assists in the implementation and monitoring of biopsychosocial treatment plans.

    Coordination of Care

    Coordinates care of patients with health care team members.

    Coordinates with UAMS staff and community to provide clinical referrals to behavioral health providers. 

    Crisis Intervention

    Is immediately available to the health care team during working hours in the event of a psychiatric emergency.

    Education, Development and Outreach

    Assists in provider, resident and staff training and education in the integrated behavioral health-primary care model.

    Develops/facilitates evidence-based treatment or psychoeducational groups as appropriate to patient/family needs.

    Performs other duties as assigned.

    Qualifications

    Minimum Qualifications:

    • Master’s degree in social work, or related field, with a minimum of two (2) years of experience as a therapist with a current license to practice in Arkansas as a Licensed Certified Social Worker;
    • Proven Competency in providing cognitive-behavioral interventions and other evidence-based or evidence-informed brief interventions to individuals, families, couples, and groups with behavioral health issues, chronic and acute physical health conditions, and substance abuse problems; 
    • Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health clinicians.
