    This position is located in Fayetteville, Arkansas


    The Licensed Social Worker will be responsible for case management plans and processes, quality control, developing processes and procedures for tracking participant outcomes and process for identifying implementation methods in maternal child health projects and programs. This person will work closely with community health workers, program manager, outreach specialists, data manager, assistant director, and care coordinators to ensure that the case management process is implemented appropriately and addresses any issues identified by community-based team members in case management process.

    Salary:  Negotiable


    • Provides consultation and management of case management process and procedures.
    • Works with team members to ensure effective implementation and care navigation and performs quality checks for case management plans delivery.
    • Provides primary care behavioral health counselling to high risk participants as needed.
    • Responsible for performing fidelity checks on educational sessions which involves relationship management with community-based organization partners performing education to participants.
    • Addresses deviations or other issues/concerns with quality of education.
    • Ensures that sessions meet program milestones and time-line.
    • Works with data team to monitor data quality and creates process improvement plans for improved care navigation services.
    • Responsible for building and maintaining strong relationships with community-based organizations, program participants, and other community stakeholders.
    • Other duties as assigned.


    Minimum Qualifications:

    • Master's in psychology, social work, counseling or related field.
    • Licensed by the state of Arkansas Professional Board.
    • Experience with Microsoft Office suite
    • Valid drivers' license required with current automobile insurance and reliable car for moderate travel to UAMS in Little Rock, AR and surrounding communities (mileage reimbursed at the current state mileage reimbursement rate) if a state vehicle is not available. Occasional out-of-state travel.
