    "This is a home visiting position and requires extensive travel in the Northeast Arkansas Area"


    The Following Baby Back Home program serves children with special healthcare needs ranging in age from zero (0) to three (3) years to infants transitioning from an Arkansas Neonatal Intensive Care Unit to their home. Enrolled participants and their families receive home visits in their home conducted by a registered nurse and SW team. 


    The Licensed Social Worker (LSW), works in partnership with the clinical coordinator to provide care and supportive services to families enrolled in the Following Baby Back Home program.  Requires flexibility in daily/weekly schedules to meet the needs of the family.

    Salary offered commensurate with experience




    • Provides home visiting and case managment services that address medical, social, emotional, growth, and developmental needs fo the child.
    • Establishes a working relationship with other hospital-based and clinic-based social workers and staff
    • Collaborates with other community resources to meet the service needs of the child and family.
    • Participates in monthly case presentations
    • Prepares and maintains accurate reports of program data and mantains compliance with all documentation of home visits and phone contacts with family.
    • Advocates for families with community entities when indicated
    • Supports the UAMS and Following Baby Back Home mission through providing home visiting caregiver education and establishing mutual goals with the family. May Service as a field placement instructor to pre-service social work students.


    Minimum Qualifications:

    • Bachelor's Degree in Social Work
    • Must have a current Arkansas Social Work License-LSW 
    • Intermediate sklls in Microsoft Office
    • Must have a valid driver's license and access to reliable transportation


    *Candidate must pass state abuse registry checks, drug screen, submit to a TB skin test and required immunizations or provide proof of immunizations. Good communication skills with adults and children is required.*
